Complete the USFAS questionnaire to find out if you are eligible to apply. Requires personal statement, student resume, a brief statement describing your current involvement (or planned future involvement) with your Native American tribe and documentation of Tribal affiliation. Accepted forms of documentation are: (1) tribal enrollment card; (2) Bureau of Indian Affairs Card (BIA); (3) Certificate of Degree of Indian Blood Card (CDIB); (4) US Public Health Services Indian Health Service eligibility card (USPHS-IHS); (5) Birth Certificate listing applicant as Native American; (6) Driver's license listing race as Native American.
All USF undergraduate and graduate level students who are of Native American and Native Alaskan descent.
